Mock fallacies and flawed logic.
Refuse accountability.
Point out errors in judgment or other failures.
Criticize tone as well as content.
Doubt wisdom, sensibility, or intelligence of a position.

ETA: Bring unnecessary side issues into a discussion.
Ignore valuable input.
Exaggerate.
Disbelieve facts.

But missing so many ways, still. As I'm thinking about it, I'm wondering if it's even possible to make a comprehensive list of the complicated ways to piss people off. There's really a lot you can do (like trademark infringement) that the root of the offense is hard to explain in emotional terms.

Some people expect you to judge their values, so there's extra stuff like irrational bias that has to go into the judgment to really piss people off.
